Jade Cronan-Thompson is a distinguished leader in the Human Services sector with a robust 25-year career dedicated to improving the lives of vulnerable populations across various sectors, including Disability, Mental Health, Aged Care, and Homelessness. Her leadership has been instrumental in strategically delivering pivotal contracts such as the Assistance with Care and Housing (ACH), Care Finders, Housing Older Women’s Support Service (HOWSS), and Head to Health.

Jade’s academic background, featuring Master’s Degrees in Social Work and Counselling and a Bachelor’s Degree in Psychology, provides her with a comprehensive understanding of the complex needs of the communities she serves. Her educational achievements reflect a deep commitment to understanding human behaviour, mental health, and the social determinants of health, equipping her with the knowledge to develop and implement effective service delivery models that address the multifaceted needs of vulnerable individuals.

With an unwavering commitment to excellence, Jade has led her teams through significant transformations, ensuring that the services provided are not only responsive and innovative but also grounded in professional and timely care principles. Precis

In response to the increasing trend of Australians ageing in place, there is a pressing need for aged care providers to diversify their business models and continuously improve care quality. This is particularly crucial in rural and remote regions where traditional models of care face significant challenges. Our program represents a novel approach aimed at addressing these challenges and improving the quality of care and overall wellbeing for older individuals in these underserved areas.

The initiative focuses on enhancing individual capacity and fostering connections for older Australians with primary care, community services, and at-home support. By employing a face-to-face support model, led by clinical and social work staff, the program delivers intensive, personalised assistance to individuals with high complexity needs. Our model is designed to overcome barriers to access, reduce avoidable hospital admissions and early entry to residential care, which are exacerbated by a shortage of services and facilities in remote and regional locations.

Older individuals in these areas often face complex barriers to wellbeing such as health literacy, logistical, and environmental concerns. Our program seeks to mitigate these issues by providing localised, comprehensive support that facilitates easier access to necessary services, both inside and outside of the aged care system.

Footprints, as a specialist provider in this field, has demonstrated expertise in catering to the needs of older individuals facing social and financial disadvantages. Their recognition as a specialist provider underscores their commitment to delivering high-quality, tailored care to this vulnerable population. By addressing the unique challenges faced by older adults in rural and remote Australia, our program aims to significantly enhance their ability to maintain independent living while ensuring their health and social outcomes are effectively managed.

Overall, this innovative approach not only aims to improve the accessibility and quality of aged care services in underserved areas but also contributes to the broader goal of promoting wellbeing and independence among older Australians.
