Concurrent session D8 – Spotlight on care at home
Thursday 2 October 2025
11:45am – 12.15pm
Precis
Home care providers are at a critical juncture regarding long-term sustainability. The coming 12 months will demand a strategic focus on enhancing service delivery while reducing overheads.
This session will explore how proactive care management, technology, artificial intelligence, and early intervention strategies can transform the delivery of Support at Home services. By engaging care workers, families, and significant others in the identification of needs, providers can improve outcomes for older Australians and ensure that available funding is utilised effectively to meet the individual goals of participants and improve financial sustainability for providers.
Featured case studies will highlight how forward-thinking providers are reshaping their service models to remain viable and responsive within the context of ongoing aged care reform. With uncertainty around future demand and profitability, it is essential that business planning includes scenario modelling and contingency strategies. Delegates will be encouraged to consider the pros and cons of new service types such as end of life and restorative care pathways. Reasonable KPIs for care partners will be highlighted as key indicators for success.
Concerns have been raised about the potential impacts of a more robust user-pays approach in packaged care. This presentation will challenge those concerns, offering a fresh perspective on the opportunities that may emerge for participants who are willing to pay for additional services. Examples of charges and service types will be shared.
Delegates will leave with practical strategies to align new care models with structured, participant-led care management, supporting improved outcomes, promoting independence and choice, and enabling sustainable service delivery into the future.
